Has anyone else experienced not having to fight the Grand Duchess (she lives and you don't judge a box) and the Guardian of Mythal Dragon (just has cutscene with intermittent dialogue) ?? Many of my friends haven't had this happen, so I'm just wondering if its just a rare thing.

Not rare just depends on what path you take. Court approval needs to be 85+ no avoid fighting Florianne. Guardian of Mythal is all about Morrigan, OGB or no, and who drank from the well.
