In cutscenes - but not in normal play - both my mage elf female Inquisitor and Cassandra have dislocated shoulders. Possibly other characters as well.
I only noticed the dislocated shoulders on Cassandra after adding arm pieces to her armour: it's possible that without arm pieces, the shoulders are in the proper place, but with arm pieces they go wrong.
For both my poor Inquisitor and Cassandra, the dislocation is essentially that the shoulder socket is empty, and the top of the humerus (with associated pseudoflesh) hovers about 5 or 6cm down and a couple of cm lateral (outwards) of the proper location.
This caused the Mark to be on the Inquisitor's wrist during the initial cutscene of In Your Heart Shall Burn; and also does rather remarkably interesting things to the Inquisitor's fingers!
After I first noticed this, I doublechecked that my graphics driver was fully updated: it is.
