Recently, I have noticed my focus resets. It'll be full three bars. Sometimes it happens right after I exit a cave, return to Skyhold or exit the radical menu.
I'm rolling a mage and it's my third run.
I'm also playing this on PS4. Anyone else having this happen randomly?

Not like you've said. I've had the whole "Quisy loses focus and gets stuck with one bar and one bar only if he dies" bug, however.
And the menu is far from radical. It's quite calm, enjoys tea for the evenings.

I don't think that game behavior is unique to any platform. I don't even use the special feature because of its inconsistent behavior. I've had both cases happen, the bar is filled or depleted on a cutscene or fast travel.
