Sometimes, I walk by some mobs and they don't react like I'm an enemy at all. I'm like: "Hello? Hit me. I'm your worst nightmare." Even when I hit them, they don't hit me back. Does it happen to some of you people as well?

Yeah, got that once. Reentering the area (coming to Skyhold and back) helps.
