Cole clearly states that Blackwall has a moment where he sees them and makes the decision to let them be slaughtered to cover his lies to his men.
He thinks, "Maker, they're young!" and continues with the assault anyway. Very gentleman like...
But no, fantasizing about what someone looks like naked does not make you a bad person. Murdering innocent people for gain makes you a bad person.

Are the people that hooked up with Bull outraged if in a subsequent playthrough where they romance, say, Sera, Bull and Dorian hook up?
I mean, what's the big deal on that front?
Your character doesn't feel as super speshul if they aren't the One True Love of their love interest. Idk, some people want to have their cake and eat it too.

I know it's true. That's the problem. I'm already having a hard enough time romancing her due to this pairing Bioware did so I don't need to think about that since it makes it harder to romance her.

Well, I can't speak for anyone but myself but as I have said before I can't bring myself to romance LIs that Bioware pair up with each other. Feels like I am ruining something by getting involved.

Man, I'm sure glad mind-probing spirits like Cole don't exist in reality, because people would be accusing me of all kinds of weird sh*t if they saw what was going on in there. Privately.
But no, I see their situations as completely different. The former was an unintentional death that was caused by self-defense. The latter was an intentional massacre that was caused by greed. And the latter doesn't try to atone for years, but just runs away. As Cassandra said, "A man who truly aspired to be righteous would not lie. He would earn respect, not steal the respect due another."
I'm not saying that the acts themselves were equivalent, only that the way it affected both of them was similar. Both were events that they came to regret deeply, and proved to be transformative for them. Both of them are people who are deeply influenced by one big regret in their pasts, and that regret comes to motivate them to tightly hold onto their own moral codes. So in that way, they're very similar. I also think that this similarity means that they'd be able to understand each other very well.

Sure, in that Blackwall's attraction is motivated by both his physical attraction and his respect for her, which in turn motivates him to get to know her better. That's how falling in love usually works.

I sensed resentment during that scene about the dog and it makes sense for a quiz who has all he wants - the glory and the girl.
If only Bioware would ratchet up the drama and gave us a way to clash with our companions as a result of our romantic choices instead of throwing in NPC fluff for the heck of it..

I wouldn't go as far as to call Rainier a rapist. I doubt that Bioware will make a character that is one, for which I am thankful.

It is a bug. Despite Josephine being romanced, the banter parts about Rainier's infatuation with her is still brought up but the part where you talk to him about it does not, thus making him appear like a stalker.

A little bit of both, I guess. It would suck for you to not be able to enjoy a romance that you seem really genuinely into, and my heart goes out to you. But these are also fictional characters. Blackwall is never gonna get with Josephine if you do the romance with her. In your version of the Inquisition universe, that relationship would simply never happen. Thinking about fictional "what if?" scenarios in an already-fictional universe is just overthinking things.
