I am in his office, after the first renovations. I've spoken to him about the lyrium and told him I supported him. I've chosen all the flirt option (up to asking him if he had anyone "special" back at home), and I am not in a relationship with any other companion. Am I missing something? I just want to trigger the first kiss cutscene! Any help would be FANTASTIC!

Hmm. It sounds like you've done enough flirts to make it appear. There should be a hearted conversation option to ask him to talk to you, in the same place where the "Did you leave anyone behind in Kirkwall" question was. That'll initiate the scene.

It seems that you did all that is necessary. Did you flirt with him right after arriving to Skyhold? In front of the gate.

Have you already had the chess scene where you tell him you should spend more time together? IIRC I did not get the kiss until after that scene played out and I was further along in the story.

Kiss scene does not happen automatically.

 

After the Cullen chess sequence, go to Cullen's Office, Click on him and if you flirted with him enough there should be an option, "I want to talk about us."

You have to do story quests to move it along. I got the kiss after 'Here Lies the Abyss'.
