I have had the same issue with the doors in the Champions of The Just & The Winter Palace giving a Perma Loading Screen
The fix for this, at least on PS4, is to read very carefully what it says next to the X button display before clicking on it. For example, if you are in the Great Hall of Therinfal Redoubt and approach the door to the officer's quarters from one angle, the X button will say "Great Hall" and not "Officer's Quarters". If you click the X while it says the name of the room you are already in, then you get a black loading screen that doesn't end. If, however, you move around a bit, eventually you'll get an X with "Officer's Quarters" next to it, clicking on this will take you out of the room as it's supposed to work. Similar for Winter Palace, don't click on the button if the name of the room is the same as the one you're already in, move around until you get the name of the room you want to enter.
I have no idea why Patch 4 added this bug, but it did, at least to my PS4 game. As frustrating as it is, you can work around it, so at least you can advance your playthrough.
After banter died in my last game, I started over, clearing all stars on the Codex entries as I go. So far, I do still have banter. Whether this is because of clearing the codexes or not, I have no idea. I'm not quite as far along as when the banter bugged out last playthrough, so it may be meaningless. Hmm, maybe I'll go to the last save in that playthrough, clear the codexes and see if banter returns.
