Both you and your spouse wear wedding rings. Your sons appearance is based on what you make both the male and female look like. So, it's a given that the protagonist had had sex in order for that to happen. During the CC, both male and female mention things like, "that [insert facial feature] is the reason why I said yes", et cetera, so it's clear both have feelings for one another. There isn't anything that I've seen that says otherwise.

 

When people bring up the issue of this, what they aren't taking into account is that, the world in the game to a completely different turn after World War 2. In the real world, homosexuality was still seen as 'wrong' and most men and woman opted to hide it and act straight, going so far as to have children and get married, after WW2. Though it's not stated in the lore, that way of viewing genders may not have changed in universe. Same thing would go for the feminist or civil rights movement. Those things might not have happened in the universe of Fallout, or maybe they happened at an earlier or later time than they did in the real world. That is what people forget when they start spouting "It's an agenda!" and all that shite. 

 

One can always argue that IVF or, well, the turkey baster, um, came into it, so sex is not necessarily itself a given, I suppose.  Alternatively, they may be a requirement to adopt within the bounds of the prospective parent's ethnicities or something.  And then there's always the possibility of playing the charade of the marriage at all times.

 

All that being said, though, there's something to be said for "in this world of the game you have chosen to play, society works like this  [points at diagram] and you'll just have to put up with it".  The setting just doesn't allow it any more than it allows playing a purple space alien (presumably) - but that's just how it's written, as with any other aspect.
