Why is it that each and every time that I start a new game I have to go into "Options" to (1.) Lower the volume on the music to 50 and set it to "Headphones"; and, (2.) Check "Hide Helmet?" Then again, there are so many things about Inquisition that are game busting annoying.

Erm...maybe a bug? I don't use headphones, so i don't know about that one, but I definitely don't need to check "hide helmet" every time I start new game.

Can't say I've come across this problem either...
I would have loved if the Hide Helmet setting (among others) was actually Character/game-specific since my girlfriend and I both play the game and having to change settings all the time, but these are both universal game settings not baked into game save files.
From what you are describing, it almost looks like all your settings revert to defaults when you start a new game?
