I was wondering if it was possible to romance Josephine up to the duel part and then choose the break up option there and then romance Cass?

It probably is, but you'd have to ignore Cass's conversations for a while, while persuing the Jose romance. I don't know for sure though

There is a certain point in any Romance where if you commits, you won't have the option to go and romance anyone else. So, I would guess...yes? Not sure though, I fully committed my Inquisitor to Cass, and never though about breaking up in the middle.

You can break the romance and persue another.

I think in theory, yes. But you can't trigger the romance scene by completing that "perfect" romance quest for Cassandra if you want to start with Josie. Still, it's super heartbreaking to pick the option.
