A standard approach is to bring the Acolyte or the Scorpion pistol as a secondary weapon just to deal with phantoms (I always bring the Scorpion as a secondary weapon on the Destroyer for platinum).
There are also some great powers like lash that deals with them.
What about an axe?
The most infuriating thing happeneds to me 1 out of 4 times. I get to play against geth

Once bubbled, dont go for the head except if U have 2 m or penetration or more. In fact, if U dont have 2 m or penetration or more, u should t make her bubble except if U have a very high burst damage weapon and U want to make her freeze3. Use projectile powers like Warp, Incinerate, etc., to make them freeze in place, and then shoot them. Singularity is probably the best, because once you drop their barriers, the Singularity will pick them up and you can hit them with another biotic power for an explosion and easy kill.
Submission net wont wk if U R more than around 5 m away4. Use Stasis or Submission Net on them.
Start the grenade throw animation away from the phantom, then come back to drop them on her8. Use grenades on them, but make sure you aren't "targeting" the Phantom when you are throwing them. Either target a different enemy, or just throw the grenades at the ground or into a corner. If you are targeting the Phantom, the grenade will have no effect.
A Marauder became invulnerable even to missile (maybe related to his melee). Tragic and comic gold fail.

nothing really makes me mad (in-game) any more...once you have played for a certain length of time, eventually..you will see almost every thing.
I was playing a silver match as a shadow against cerberuse on Firestome Jade. I cloaked up, and used shadow strike to quickly get across the map to insta kill a phantom, except she ignored my cloak and insta killed me in mid animation of my insta-kill against her!
Why did this happen?
Also, I would love some general advice about how to kill phantoms without a shadow. I can't ever kill a single one without hiding in stealth. Nothing I do will stagger them!

^ I know the footsteps are real, but I didn't buy it that it was the case for SS.

Killing Phantoms?
1) Acolyte to strip shields and then any biotic power that takes her off her feet: singularity (followed by a detonating power like warp or throw or shockwave to explode them), throw, lift, ...
2) Dark Channel is fantastic. Can kill her on her own, but also primes Phantoms which you then can let explode while they make their backflip with any detonating power.
3) Another power combo not named yet and one of the best, if you have the N7 Paladin: snap freeze and energy drain. Can kill phantoms in one cryo explosion. If a phantom isn't dead then, you can always follow up with a shield bash if you at least got her shields down. Nothing feels better then meeleing a Phantom to death.
It's probably cuz OP cloaked right in front of her or she watched him cloak a few feet away before shadow striking... ..she will just automatically go into her sync animation...sux but that's how it happens....It's hilarious if you do that to a Guardian...by the time you cloak and hit shadow strike he will do a 180 and block it with his garage door...
I do miss the good ol' dayz..back when we could shadow strike an Atlas

Agree with FTL though shadow strike works better if you just blow their ****** ****** faces off first.
