Always:
Save the Rachni Queen
Save the Council
Save Kaidan in Virmire
Destroy Collector base
Everyone survive the Suicide Mission
Cure Genophage
Use Renegate option on Kai Leng
Achieve peace between Geth and the Quarians
Choose Destroy
Never:
Recruit Morith
Got Tali exiled
Let Garrus kill Sidonis
2) Refuse ending. Okay this is probably going to annoy some people, but to me this ending just feels like killing everyone. And I while I'll be willing to kill of a few of my favorite ME characters in a particularly renegade playthrough I can't bring myself to get all of them killed.
I don't even consider the 'refuse' ending a proper ending. I seems more like a slightly cruel in-joke from the devs to people who talked about 'telling the star child to go %$£? himself'. I'd only pick the 'refuse' ending if it had, say 4 different outcomes ranging for terrible death and destruction to total victory based on your EMS.
I also never pick 'destroy' as that (to me) renders the whole 'synthetic rights/organics vs machines/are the geth really evil?' underlying message of the whole trilogy to be completely pointless. I also can't be 'nasty' to people in ME. If someone has a problem and the only choice options are essentially: 'you stupid b*****d' and 'oh, you poor dear!', I have to pick the latter even though I personally wouldn't say either extreme.
Never:
Pick Quarians over the Geth. I can't bring myself to help someone who doesn't want to be helped. Although there's the peace option, so I don't necessarily have to let the suicidal conformists die either.
Always:
Kick Admiral Gerrel in the nuts. Bastard deserves it.
Also I don't think I could let Wrex die. Even if it means Mordin can live, I wouldn't want it on those terms. I feel bad enough for putting Wrex into the Virmire situation!
