A LOT of people have been asking for this and I was finally able to finish it up.  Below you will find links to videos that will help you track down the pieces missing from the golden mosaics on the the walls in Skyhold.  In the videos I have mapped each piece from the golden mosaics on the walls in Skyhold to videos that show you the locations to go in order to find each piece.  I really hope this helps everyone track down those final few pieces that you may be missing.  

 

This has been verified across two playthroughs on the PC as well as one on the XBOX to ensure accuracy across playthroughs and platforms.  

 

The Archdemon Mosaic Collection 

The Archdemon is on the right wall if you're facing the door out of Skyhold keep.

 

The Fall Mosaic Collection

The Fall is on the left wall up by Vivienne.

 

Freed Are Slaves Mosaic Collection

Freed Are Slaves is on the right wall up by Vivienne.

 

Invasion Mosaic Collection

Invasion is on the front right wall if you're facing the door out of Skyhold keep.

 

Sacrifice Mosaic Collection

Sacrifice is on the front left wall if you're facing the door out of Skyhold keep.

Thank you, that does make it a lot easier to see which pieces are missing, then locate them.

Thank you, that's going to be very useful. I have a few sets missing pieces in all of my playthroughs.

Ahh thank you!!! I have never completed the mosaics because I didn't fancy running to all places where the final one or two pieces might be. This way I can just go and pick up those I missed, awesome!

Final piece is now in the wolf den. I have not checked the one by the upper left dungeon as I had already made the jump for it, so I'm not sure where it was moved but it should be in the area.

#10 for The Fall was fixed as well.  It is further down into the vielfire cave after you find #9.  The videos have been updated to include the recent changes for #10 and #12 from Patch 6.
