If you'll recall, The smaller Fears that we fight in the Fade appear differently to each person. Both the Inquisitor and Hawke see spiders, Sera sees blobs of nothingness, Blackwall sees Darkspawn, etc.
What would your Warden see?
My Aedan Cousland who romanced Morrigan and did the Dark Ritual would see one of two things:
Option A: Bears.
No, not these ones.
These ones.
House Howe's heraldy.
Option B: An evil Kieran reveling in his godly powers.
Clearly my Cousland would see giant rats, still better than what Cassandra saw the fearlings which made me not want to know what she saw the nightmare as.
Since my warden has a huge guilt because he did a lot of bad things to ensure victory since he knew that he couldn't fail, he would probably see all the innocent that he killed
my warden seeing him with a psychotic smirk on his face after he killed Rendon Howe. Yes it is a nightmare-ish and twisted and ****.

Red eyed, emaciated, very intelligent and malevolent looking demon rats would work.
Really though, apart from that, the way to throw her off would be to show her visions of that night in Highever. The servants, the Knights, Mother Mallol, Nan, her parents...all dead.
Him playing Dragon Age 2 for eternity.
Ghoul-ified Tamlen. Or just ghouls in general. Of everyone she cares about. Leliana, her friends, Wardens. It's twisted the things she cares for most, forced her to kill them by her own hand, and it's also a fate that's possible for herself. Which makes it all the more distressing. Could also represent all the people who may have still been alive in Amaranthine when she burned it to the ground.
Broodmothers, as well. I imagine a lot of female Wardens would be more than a little shaken still. I know mine would.

I'd go for the obvious answer of having failed loved ones (Shianni, Leliana, Jowan if my Surana hadn't thought he was such an idiot for getting caught) but Broodmothers are just really....nope.
Oh, Creators, yes. My Wardens would see Nightmare as a broodmother and the spiders as those freaking childers. Being overwhelmed and having them crawling on you...[shudder]
This is probably one of the obvious, and cliche answer. I'd still go with it, though, as it fitting my Warden most. My Aeducan would see his father Endrin on his death bed, blaming him for his failure, and say that he's a disgrace to Aeducan's name. Then he would see Orzammar falling apart, and his people slowly die out after a years of siege, the Darkspawn finally break into the city and kill every Dwarves, but spare the last dwarf, who's a women, only to turn her into a Broodmother that would spawn a relentless wave of twisted being that mirroring his people swarming him. In his nightmare the surface civilizations not only refuse to help Orzammar, but seal its gate tight so no darkspawn, and even dwarves can flee to the surface.The nightmare would end with him, seeing Flemeth enslaving Morrigan, and turning his son Kieran into a monstrous being that would bring the end to this world, and he's forced to kill his own son and lover.
My Mahariel would see a medley of things: a ghoul Tamlen to remind him of his failure and the fate that awaits him, Kieran turning out to be evil and causing apocalyptic destruction, Leliana going full zealot and leaving him for good (He's very insecure)
Probably spiders, realistically speaking. They seem to be the "blank" state of the Nightmare's brood, projected to those whose mind can't be easily read. Given that the Warden also had a generic, non-tailored scene created by the sloth demon in Origins, it seems the Warden has the same advantage.
That bioware will touch him and will twist him into something completely different than he is what sadly to exent already happened.
or prob his former party members cus he was tired of their unhealthy issues and crazy s**** .
My Queen Cousland would see the death of Alistair/all those she grew to love during the Blight. She would also see Nan...that woman is the stuff of nightmares.
I'm most certain about my Queen Cousland, who would probably see little naked Morrigans scuttling after clueless little Alistairs.
