Gameplay wise, I'm always using Jack / Miranda as a Soldier. Miranda is probably the best squad mate because she's useful with everything while Jack has Shockwave (and I can never get too much shockwave).
If I was to create a squad though, I'd always tag along my love interest and whoever else (although it tends to be biased towards Garrus).

Legion, for his survivability, wicked sniper rifle, and drone/AI hack.
Garrus most other times if I need an ammo power, plus his overload and sniper rifle is usually quite useful, especially since I picked up the Incisor.
These guys also happen to be my favorite for story purposes.

Miranda - always performs.
With the Mattock now available I find Samara even better than she was before (and I used her a fair bit) so I often have everything I need with Samara and Miranda.
- Kasumi is always useful either with Overload or ShadowStrike or Flashbang.
- Any of the snipers (Zaaed, Garrus or Thane) with the Incisor makes a mission easy.
- Lastly, Jacob or Jack for Biotic plus shotgun - they are consistent with their ammo power making them useful.
- Honorable mention to Mordin, Grunt and Legion.. never had real trouble when I have them in the team.
I can't say I use Tali much.
I like mixing up my teams and with the number of upgrades, new weapons and new armor now available.. not much of the game can't be trashed quickly. (The mattock really is an upgraded Revenant/Sniper rifle for everybody)

Personally I found that Kasumi/Thane, Kasumi/Garrus, or Garrus/Thane are good squadmates to take with you on missions. Especially if you have their loyalty.

Miranda and legion gameplay wise
Legion can pretty much take down anything with the anti widow and mirandas boost
Insanity was way too easy towards the end

my fave combi was garrus/thane - altho i was always enjoyed putting mordin with garrus/thane/grunt for missions that didn't involve fighting mechs or geth

The team i used all the time on my first playthrough (and like best) was zaeed and miranda.
Miranda: Nice to look at, good all-round character, handy in all situations, DEADLY with tempest.
Zaeed: Awesome. oh, you want more? well, put a mantis in his hands with disruptor and he tears through hordes. Inferno grenade is also funny and amusing

My number one squadmate is Miranda, hands down.
She has warp and overload. Nothing is safe from her. She benefits from both tech and biotic upgrades, also.
I never fully upgrade her loyalty power, Slam. It's a great power, but you can fully upgrade everything else first. Why wait until you've unlocked her loyalty power to get a powerful mix?
Throw in the bonuses she gives the squad too.
On a related note, I never get Heavy Warp. I always get Unstable Warp -- because Unstable Warp recharges in 9 seconds rather than 12. The math says that you can attack four times in 36 seconds with unstable compared to 3 times in the same time period with heavy, and that doesn't take upgrades into account.
The rest I'm putting in alphabetical order:
Garrus
He has Overload. He have concussive shot, which is good
against barriers. He has a bonus of up to 25 percent on weapons and on
powers. Give the guy a Geth Assualt Rifle (hardcore and above) and an Incisor (DLC) plus team warp ammo and watch
enemy shields and barriers disappear.
Grunt
Grunt requires some micromanagement because he charges everything. However, his shotgun combined with high health make him great when swarmed by husks and melee critters. As a soldier, I've killed many a charging Krogan who was locking horns with Grunt.
Jack
Weak against shielded and armored enemies. Requires her own fully upgraded Warp ammo and the Geth shotgun (DLC) to come
into her own. Requires some micro-management because her
shotgun-wieldning programing has her leaving cover and closing with the
enemy all the time. Team her up with a shield-killer like Miranda or
Garrus, though, and her Shockwave is devastating.
Jacob
Another
squadmate you can develop very fully before he gets his loyalty power.
Frankly, like many players, I find his loyalty power annoying. Unless
you do everything yourself, he's "throwing up a barrier" every time you
want him to pull something. Incendiary is very nice, though, particularly early on.
Kasumi
Here flashbang loyalty power is easily unlocked early
in the game, and very useful against all kinds of enemies protected by
all kinds of defenses. Not everybody finds flashbang as useful as I do,
however. As a soldier, I can shoot up anything that's stunned, however
briefly.
Legion
Most useful against synthetics, but a good strong shot
with the Widow and with a good assualt rifle against any enemy.
Requires a bit of mico-management to get the full benefit out of the
Widow. Have Miranda or Tali strip away shields and his hacking power makes him the go-to sqadmate for fighting Geth.
Mordin
Mordin's best used against organic enemies but even with upgrades, his incinerate power takes a bit long to recharge. If teammates strip away protections, Mordin can cryo or shock enemies at such a fast rate, he can seriously disrupt enemy attacks. His weapon damage bonus is not bad, either.
Morinth
Never used her.
Samara
Weak against shielded enemies, but has very fast recharge of her boitic powers. Her loyalty power of reave works very well against barriers and armor. Give her a Geth Pulse Rifle and her performance against shield is satisfactory.
Tali
IMHO, Tali's underestimated as a squadmate. Energy drain
is better than overload, although it is slower to recharge. Drone is
useful as a distraction even against very powerful enemies. She's
definitely one of the chief beneficiaries of the Geth shotgun in the
DLC, especially if you give her squad Warp ammo.
Thane
I used to gripe about his shredder ammo loyalty power. Then I realized that his "Drell Assassin" power means he has an ammo bonus for ANYTHING he fires. Give the guy squad warp ammo and put him behind decent cover with a good field of fire. He's a one-man army.
Now give him the Incisor sniper rifle (available through DLC) and put him on the same squad as Miranda, with her team weapon damage bonus.
All this and unstable warp too.
I'm on the last fight in Samara's recruitment mission (Insanity level) in my latest game. We just move up to the second, higher room. I mess around, get a canister of toxins thrown at me and have to leave cover. I bungle the move and get seriously wounded. I'm crouching behind cover and, before I can heal up, Thane and Miranda killed every enemy in the room.
Zaeed
A weapon damage bonus as good as Thane's, plus an inferno grenade loyalty power that's easily unlocked early in the game.

combat - Grunt, has alot of staying power even on insanity
personality - Mordin, IMO the deepest character with a good lineup of morality choices and an excellent voice actor
