Alright. This is my second play-through. During my first play-through his Revelations quest triggered without a problem. The second time around in my now play-through it has yet to trigger, unfortunately I'm near the end of the game and it still hasn't triggered. His approval rating with me isn't very high, at least I don't think it is. I don't use him for quests or anything, but whenever I make a decision I usually earn his approval which is what's confusing for me. 

 

During my first play-through it triggered after the battle at Adamant, but I'm far past that.. I exiled the Grey Wardens, but kept him, could that have caused the problem?

It might cause an issue with his approval. I believe it's an approval threshold that actually triggers the quest. You could take him along to find any Warden artifacts you haven't picked up yet.

Yes, it has to do with his approval. If you exiled the Wardens this time, it will take a little longer to compensate for the approval you lost. Collecting those artifacts is a good idea so long as you make sure he's in the party when you find them.

It has to do with approval. I'm in my second playthrough had the grey wardens exiled, and empress celene die so it plummeted his approval. I think they should have it trigger without approval since it's a pivotal part of makes Blackwall a well written character.

Killing Darkspawn in Storm coast and Western approach with him gives approval too.
