You can get a pretty decent look while at the ball at the Winter Palace. Only time you can see it though, since that's the only time she's out of armor.

Meh, probably. I didn't notice it very much either way, but that's the only time that I can think of. Leliana is set to be in a relationship with my living Warden for this current character's world state, so it doesn't phase me either way. <shrug>

Apparently, during the Templar quest, when the demon impersonates your advisers, it was originially written that Leliana (well, the demon) tries to seduce the Inquisitor and it was possible to actually sleep with her/the demon. Bioware allowed play testing for the templar quest, and some female players found it offensive so they scrapped it.
This is what Lady Insanity stated in her Assquisitor playthrough
