Many games like the tout the story as one of their major focuses but few get it right, what are some of the best examples of video game storytelling you can think of? What are some of the best stories to be told in the video game medium?

I think the Bioshock series did a good job of storytelling as well, especially for a first person shooter series.

Oh, and Looking Glass + their run off studios were absolute masters when it came to storytelling within games. Ultima Underworld, System Shock, Thief, Deus Ex.

The original Homeworld.
A compelling premise combined with clean, minimalistic cutscenes and dialogue made for an engrossing single-player campaign.

My list, but no particular order
Planescape: Torment (but there shall be no "best video game story thread" without it)
Gabriel Knight Series
The Longest Journey
The Walking Dead (Telltale)
Neverwinter Nights
Witcher
Final Fantasy VII
Mass Effect
Dragon Age
There are so many well written stories out there: some are heartbreaking, some are inspiring, and some can not describe better than a masterpiece...

Clearly this is the Greatest Video Game story ever told!
All those other stories wish they had even the tiniest fraction of the depth, and emotion of this story.

I really enjoyed Red Dead Redemption's story. Rockstar did a very good job of integrating its story with the open world.
Especially the epilogue.

I've heard that Planescape torment is great at telling a storytelling but I couldn't get into the game since my old PC broke.

Just going to write 4 examples that stuck out for me.
Silent Hill 2 for being able to convey the inner psycological struggles of a broken clerk salesman named James Sunderland by using very little amount of dialogue, and instead utilize the atmosphere of an abandoned seemingly haunted town, music and environmental design, in order to siphon the narrative subtlety to the viewer through metaphorical and symbolic context.
Persona 3 -- Being able to tell a very engaging story about a cast of young high schoolers in Japan's struggles to come to terms with their own mortality, while they fight to prevent mankind from smothering itself.
Metal Gear Solid 2: Sons of Liberty -- Post-modernism's entrance unto the video game stage. A story that centers around the meaning of identity, memetics(Richard Dawkins' term referring to tunes, ideas, catch-phrases, clothes fashions, ways of making pots or of building arches), Global control via Information control and legacy/how one may leave their mark when their time is up. It does this by deconstructing the common tropes of a spy story and delivering one of the most through provoking narratives to be seen for years.
Knights of the Old Republic 2 -- A powerful subversion of the average Star Wars tropes, while telling a self-reflecting story of a Exiled Jedi, and her/his struggle on coming to terms with ones own past.

The Last of Us- Truly magnificent storytelling. Following two very different people who bond with each other in difficult times, going from indifference, to acceptance, and eventually learning to love each other as a father and daughter type relationship. It's poignant and isn't afraid to show things others wouldn't. It pulls no punches, and doesn't provide a black and white morality. The protagonist isn't a great human being but he isn't an anti-hero either. He sometimes does the right thing for the wrong reasons and the right thing for the wrong reasons (what he does leading up to the endgame is a great example of this) Ends on a very bittersweet note, but one that left you feeling satisfied with the conclusion of the story, and not wanting a sequel to tie up loose ends.

Neverwinter Nights
Why Neverwinter Nights? I'm guessing you listed it for the player made modules. But if you mean the oc then you're crazy, it was very mediocre, hardly anything to write home about. Many of the player-made modules have better storytelling.
I have to admit though that the cut original plot for Neverwinter Nights sounded amazing, stupid atari ![]()

I listed NWN as I liked it a lot: the whole game with the expansion packs, and including player-made modules.
As it is a subjective list, I did not think everybody will agree with it

Halo series is one of the best modern Sci-fi franchises that is constantly expanding its lore, I'm personally looking forward to see were it goes in terms of story telling.
There is also the Half-Life games with Half-Life 2 imo being the greatest game of all time and has excellent story telling unfortunately Valve are letting what could be a great franchise be quickly forgotten (also they need to count to three).
Both KOTOR 1/2 had some very well developed stories though imo 2 was the better one, unfortunately they'll be no KOTOR 3 and both the exile and reven were ruined in the revan novel and TOR.
Bioshock also has very engaging stories with the 1st one being the best out of the three game released, Mass effect also has an vats universe like Halo but in terms of story its usually hit or miss.

I honestly believe that Halo is the only good FPS storyteller (I shed a manly tear at the end of 4).
Also, SPEC OPS: The Line. Easily the best deconstruction of the shooter genre.

Dark Souls. The narrative itself was awesome, but what's more the style is something that could never be repeated in any other medium. It's a world built with a great deal of precision, at least as much as a game world can be.

The games that stuck out the most to me for their story telling are...
A)Spec Ops:The Line. Incredibly disturbing, visceral horror. The greatest deconstruction of military shooters and American foreign policy.
The Darkness. THe graphics are pretty awful to look at nowadays, but it also has the most tearjerking moment I have ever seen in a game.
C) Deus Ex: Human Revolution. THe character work is phenomonal, the world fully realized, and plot is a top notch thriller.
D) Planescape: Torment. 'nuff said

Best stories, or best storytelling?
Bioshock Infinite has some of the best storytelling I've ever seen in a game, though the story itself has major issues.
