Anyone else have rendering issues?

 

I play on the PS4.  I have dubbed this my "Thalidomide Armour", because of my empty arm sockets (note the distorted hand):

 

 

Here is my Inquisitor with her disarticulated arms and missing waist:

Towards end game my Cassandras shoulders vanished never to be seen again, it was quite weird.

 

Look closely at Cassandras shoulders in my screenshot.

My first inquisitor's vanguard armor was missing its waist section. It looked like he'd just escaped from a creepy stage magician.
