In all my playthroughs, that war table mission has triggered once. I've tried every single combination, race, gender, class, supporting mages, templars, the only thing that I've done the same all times is the decision I take at the end of the Demands of the Qun, that as far as I know is the main requirement for this mission to trigger.

Did you go and speak with Bull again in the tavern afterwards? The scene is a cutscene you get from chatting to him after the quest is over.

It's not a war table mission. It just shows up as a cut scene after you decide to save Krem and Company rather than side with the Qun. So it would never show up if you didn't agree to go with Bull to the Storm Coast to help the Qun. I'm assuming it wouldn't happen if you sided with the Qun. I've never done that, so don't know for sure, but I don't see why they would send assassins if you sided with them.

It wouldn't - the assassins are a message to Iron Bull, so he has to save his Chargers for it to happen.
