I know this isn't so much of a "quest", but wasn't sure where else to put it. To start off - YES I know there is a level. Problem I encountered was that when I pulled the level to open it I also clicked on the little point of interest/claim point right next to it. I'm assuming this somehow interrupted the opening of the gate because now it is showing that the lever is pulled down but the door is not open. I tried teleporting to another camp and teleporting back but that was a no go.

 

Has anyone encountered this issue or know of a way to fix it? It's a bit trivial but it's going to annoy the CRAP out of me if I can't open that door now because of something silly like that.

That is odd because the lever for both doors is distinctly a few paces from landmark markers and it's kind of hard to hit both, door lever and marker, at the same time. You said you tried porting. If it's within the same map, it may not work. If you tried porting to other maps and back again, try repairing the game. If the glitch remains the same, then there's nothing to be done.

Got to the old prison, and took the elevator down to Corvincius (sp???) but now I can't get to the East side

there is a path that takes you all of the way through the prison and out the other side. On the other side is your final inquisition camp you can set up.

The exit from Coracavus is in the area where the giant roams. Once you set up the final camp, you have to kill two more giants, open those two Toth doors and so on.

found the exit- don't know how I missed it (TWICE!) , maybe playing too late at night

These doors always seem to Lol me because I think the same thing everyone is thinking but the process turns out to be different and a loooooong process.

 

In this case in the western section of WESTERN APPROACH you need to do a WAR TABLE mission to clear up the toxic air. Once that's done you can progress north and follow a mountain path up and east. It will take you to a door. Enter and go through the mini dungeon (don't forget to close the darkspawn hole). Kill the giant and exit the exit door. head south east. Grab the camp and quests if you want. keep heading southeast and you should reach this same door in question I believe there should be a switch nearby. hit the switch to open the door. Congratulations! You did a-lot of crap and now just realized that you opened the door in question. Lol.

 

This game has a nasty tendency of making you pull a RAGE ( Start             Finish) by making you make giant C's by starting here at one point and ending near where you started.
