Please, no more Varric. I like him, but that's enough and it wouldn't be fair to the other companions if he's the only one who keeps re-appearing.

They've never had a romance option be the carry-over companion, so I doubt it's either going to be Dorian or Sera. Vivienne can possibly be Divine so she's probably not going to be the one either.

This is the right way to approach this, I think.
So we start with: Cassandra, Solas, Varric, Vivienne, Sera, Blackwall, Bull, Dorian, and Cole
I doubt that they will bring back a romance option. Too many people are going to be miffed that their LI isn't with the Inquisitor (assuming a new PC). They especially won't be bringing back Dorian, Sera, or Bull because that would mean that there would be even more LGBT companions in the next game than in this game (returning companion + new options) and I suspect people's head will explode because of 'teh agenda'.
So now we have: Varric, Vivienne, and Cole; Cassandra, Solas, Sera, Blackwall, Bull, Dorian
Vivienne is also a no-go because she can potentially be Divine, so there's no way that she's going to come back.
So now we have: Varric and Cole; Cassandra, Solas, Sera, Blackwall, Bull, Dorian, Vivienne
These are the two where it wouldn't be overly complicated to bring them back. But Varric's already been back twice and we've gotten some good closure on his storyline, so I think it's time to retire him. I honestly can't see him coming back.
So that leaves us with: Cole; Cassandra, Solas, Sera, Blackwall, Bull, Dorian, Vivienne, Varric
Now, I'm fully in the camp of people who think that it's not actually going to be a returning companion, but a returning semi-significant NPC. My money is 100% on Harding making the return. She's a fan favorite; she's a capable ranged rogue; she's a possible bisexual romance option (giving us our first dwarf romance); and she's easy to fit into any upcoming story. It's as simple as a cutscene where the new PC is told, "The Inquisitor is indisposed of with his/her other duties, but s/he wants to show you support. Please accept the services of this agent as his/her token of support." <cue Harding>
If we go north, it'll be Dorian. Even though he's a possible LI for the Inquisitor, he's too closely tied to any Tevinter shenanigans to disregard.

Plus, I think that his role (idealistic, reform-minded Magister) is going to be played by Maevaris. Gaider has already stated that he loves her character and wanted to fit her into DA: I, but she was created too late in the process. I have a feeling that she'll be our "good Tevinter" mage companion if we go up there. And, since they've already established that Dorian and Mae are allies, that's a great way to include a Dorian cameo and introduce Mae.
I never liked the carry-over idea, with the exception of Oghren, but her was from a game to its expansion, not to a new game altogether.

Not counting side characters, since it seems legitimately possible that we'll see Scout Harding again:
Cole. He's such an easy exposition character, and we likely haven't seen his full character arch yet. I won't be surprised if we see him in the party again.
I hope Calpernia, she was really interesting, and seeing we might go to Tevinter next. Also I read that the writers really liked her as well so that might up her chances.
Signs do seem to point to Varric as well but I really hope that won't be the case. I like him, but having him as a companion for three games in a row would be far too much. It would feel very much forced.
As far as companions go, I think that Dorian has a good chance of appearing again. If not as a companion, then as an important NPC since Tevinter seems like a good possible location for the next game.
In regards to minor NPCs promoted to companion status, all I know is that I definitely do *NOT* want to see Bianca anywhere near the companion roster.

I think it will be Varric again. We haven't resolved either the Bianca storyline or the Red Lyrium, so unless it's resolved via DLC I think he'll be back. Also, Brian Bloom voices the DA Keep. LI's don't return so unless they change that it's basically Viv or Cole. Viv has the possibility of being Divine, so she's probably a no-go, and Cole has two completely seperate paths which would also be difficult to implement.
So... Yeah, we'll probably be seeing Varric again.
