i'm level 19 and done pretty much, like getting finest armor and weps, then i noticed that I missed blackwall and sera and that ironbull........ I always loved mages, so viviene and dorian was enough for me. Also cassandra was good tank. i didnt have any problem on party combination. But i needed something new and you know..... but then...missed 3 character. i feel like i lost so many things. i heard SERA is very funny character and with blackwall they are comedian. And ironbull is also badass funny guy. I wish bioware makes available they unlock quest once again. It's just too much for starting over.
After playing 40hours....

Blackwall is quite easy to miss since his recruitment is timed. I don't think that's the case with Sera and IB, however. You might still be able to recruit them.

This happened to me the first time I played DA:II. I missed Fenris. Kept looking for him all through the game and never got him. Found out later that I needed to do that quest in Lowtown. Same with Isabella the second time around. I didn't go to the Hanged Man at the right time and she was gone--didn't really miss her except for the lost experience points.

Just once.
I really enjoyed my first 40 some hours playing but truthfully it took me that long to get a good handle on a lot of the games options and features, plus the controls and special abilities so I've enjoyed playing the next 40 hours a lot more than the first. Not a criticism you understand, the game is complex and it just takes a while to learn enough to really start taking advantage of some of the special abilities, etc of each class. But that's why I'm still having a blast with it at 400 hrs plus I would imagine.

In the beginning at Haven, you have to speak with Krem after going to Orlais, I think. It's probably possible to miss recruiting him entirely if you don't once you've reached Skyhold. I've missed recruiting Sera once. So far, Sera was the only one I missed before and that was during my first playthrough.

My first PT was very fast, I missed several areas altogether, but my canon PT was over 150 hours.

I'm at hour 50 and I'm level 5....
The perils of hunting for screenshots.
I can't imagine not picking up all the companions, but then I'm a sucker for content. First priority for me in this (my second) PT was to pick up Sera. I didn't have her around much last time (despite romancing her) so I want her in my party plenty this time. She's also incredibly photogenic, which helps.

In DAI, they pretty much feed you hints and quest of who are the companions to recruit.

I don't know, it was pretty easy to miss Sten and Leliana. My first run through origins I pretty much did only what was needed in Lothering and didn't even know about them until near the end of the game when I went online to look for hints and such. DA2 Fenris was easy to miss because his quest was only optional as was Isabela's but in her case she was right in your face.
DAI is a lot harder to miss them as they are pretty much thrown at you from the start. Though saying that I had Iron Bull's quest in my log until well into getting to Skyhold, I just overlooked it in my log due to so many other quests. Sera's and Viv have to be the easiest as you are right there in the city when they are given to you.

I spent most of my first PT just trying to figure out how everything worked, Inquisition was the first game of this type I've ever played. Really enjoying it especially now, my current characters include a Dwarven two handed rogue that is just a blast to play

While nothing can beat the first play and seeing everything untainted by previous experience or meta knowledge, I always find my successive plays to be more enjoyable for this reason.
