Ah I didn't revealed it twice and it's not showing up!
I have the same problem with a lot of my tiles in addition to some displaying incorrect outcomes (e.g., Leliana being Divine instead of Cassandra). We'll just have to wait until they iron out the bugs and remove the hidden tiles I suppose!
In my keep, in companion section, there is, "did the Inquisitor find source of red lyrium or not"? In Varric's section. Only the option "yes, did find the source" is revealed to me. Underneath that, it asks me whether I was friends with Varric or not. You have to be friends with Varric in order to get this quest, because it's his companion quest. Yet, "Inquisitor was friends with Varric" is hidden but it's still been automatically selected by the keep!! Wtf? I know because it has the white outline around the square. If I did the quest then that means I was friends with him, yet the option for that does not come up. The Keep is contradicting itself.
I also have "iron bull became tal-vashoth" I know this is 100% wrong. I never got Iron Bull's quest, I barely even spoke to him after recruiting him, I found him annoying and avoided him like the plague. The other section though, whether IB saved the dreadnaught or the chargers, is also hidden. But if the option that IB became tal-vashoth is revealed, why isn't the corresponding quest tiles? The whole thing gives me a headache.
Something even stranger happened to me - the option of whether IB saved the dreadnought is unlocked, but the only available tile in the corresponding section states that IB became tal-valshoth. Clearly these two things are entirely inconsistent, as the Keep itself recognises when it tries to automatically change one of my choices...
I'm not sure why the team are so insistent on keeping the hidden tiles. There are far better solutions to masking or warning about spoilers and it seems a lot of time spent solving bugs related to the hidden tiles could be saved by simply removing them!

Actually, that raises the question - why ask both if you saved the Chargers, and if Bull became Tal-vashoth? The choice to save the chargers is what makes him Tal-vashoth, and the Keeps description for the "Saved Chargers" option even says the Qunari outcasts him from the Qun. 0o
I also have a lot of hidden tiles and I just completed Inquisition and found that within Dragon Age Keep that character does not exist, I have 17 others, including deleted characters and a couple of duplications but not the one character, so far, that has finished the whole game. I hadn't checked with Dragon Age Keep since I started this play through but when I confirmed the game state when making the character the Inquisition portion had not become active.
About the Inquisitors in the Heroes section: I read that they appear only if you're connected during the CC. This isn't possible, because I had 3 Qunaris (2 attempts and 1 "official") and after an update, my official Qunari was gone, while the other two are still there.
I have no idea why I should be connected to the Keep during the CC. This means that, if I lose connection for any reasons during the CC, I won't be able to see my Inquisitor??? Well, s***!
I have yet to unlock the recruited Cole and Dorian tiles even though I have. I have just unlocked Dorian's personal quest tile though. And I can select it, which is weird because apparently this means recruiting Dorian isn't necessary to completing his quest, despite the fact the Keep fails to recognise the ability to complete Solas' quest and punch him, (which I've just done).
It keeps telling me that I'd create an invalid world state if I select that I gave Vivienne a snowy wyvern heart (I did) but also try to lock in that I was not friends with her (I wasn't. I hated her.).
I also have some other hidden tiles issues. It will let me say Fairbanks solved the Freemen issue but nothing about his heritage. Most of the ones underneath Exploring Thedas are also hidden except contacting Charade.
If there are still ppl with nearly everything hidden or some hidden and you've completed the game I suggest trying this and seeing if it works to unlock some for you.
I got a few unlocked by selecting "replay start up" from the drop down menu in the dragon age keep and waiting for it to sync, should be complete when the 3rd circle shows a check mark, there are finally some of my choices unveiled to select in the tapestry now. I'm not sure it will help anyone else unlock more of theirs but it definitely unlocked a few of mine so I figured I'd share in case it could.
^Are you sure Vivienne hated -you-, though? It was my understanding that if you give her the snowy wyvern heart that you get a massive boost in approval with her; ensuring that you are friends with her from then on.

Tile for being friends with her is still locked. That way, one of the tiles has to stay empty, which is rather weird. Perhaps it's a bug where you have low approval with her and complete that quest, but game somehow fails to acknowledge that you are friends with her now.

I'm having this exact bug, and I won her approval through the wyvern heart as well.

The lovers happens after you complete WEWH or the grey warden quest.

I don't really feel bothered by this because by the time we actually need to import the Inquisition Keep state into everything, they will have removed this hidden tile business.

Kantr's link above shows how you can access the quest in-game. If you just want to see it in the Keep, go to your Settings and turn the spoiler toggle to off.
