Just a quick question, how do I add gems and other stuff to gear that has slots available ?  Thanks.

You don't equip gems at all.  You have to equip arm/leg modifications in armor, and runes/multiple other things on weapons.  These can only be done at your forge in Haven/Skyhold, or one of the forges at one of the 3 Keeps
