I'm not sure if this is the right place to post this, but I didn't see a general "bugs" section...

Quest: A Friend of Red Jenny (Sera's Recruitment)

--The Bug--

On a couple of my playthroughs I experienced a bug where Sera, when first recruited, arrives with no equipment at all: no armor, no weapons.  A google search found other players with the same problem, but no fixes.

--The Workaround--

On a hunch, I reloaded an earlier save and I think I may have found the pattern: After you agree to let her join the Inquisition, you are immediately presented with the companion-selection screen.  If you include her as one of your party NOW, there's no bug: she gets all of her equipment and keeps it.  If you instead leave her benched (choose, say, Cassandra, Varric, and Solas), it will trigger the bug: when you return to Skyhold and check everyone's inventory, you'll find she's missing her stuff.

So put her in your party as soon as you recruit her!  I haven't tested this exhaustively, but it seems worth a try :)

Judging by the threads here in BSN, Sera is indeed missing her stuff...

More seriously, companion starting equipment should be replaced ASAP anyway. Sera's crappy armor and crappy bow make no exception

That is also the case for Blackwall and Iron Bull. I would say it's intentional, if you add them to your party immediately, you get their starting gear, if not the game assumes you want to change the gear before taking them out to fight so, it doesn't give you the starting gear.

Thanks for the heads up; I never noticed it with those two!
