When you go up the elevator near the start, Shep goes "these look like gang signs" or something and Aria mentions the Talons. At least 50% of the time, for me, these graphics dont load at all.

The final romance scene, the one right before Cerberus HQ.
That reminds me, there are some dialogue sync glitches during the final romance scene with Liara.

Mass Effect 2: Enemies will not always be lifted off the ground by biotics such as Pull or Slam.
Mass Effect 2: Various charge bugs where the charging animation plays but Shepard does not charge, and the cooldown still starts. This can also sometimes lead to your shields not recharging.
Mass Effect 1: During Benezia fight, if knocked down and then a Benezia casting cutscene starts, you will remain on the ground. Sometimes you turn into stretchy man/woman.
Mass Effect 2: Game will sometimes make you float up into the air when backing up next to cover. NOTE: I believe I read that this is a common known bug with Unreal Engine and may not be fixable.

Mass Effect 3: Similar to the "invisible Rannoch rock" (which has never bothered me), Dr. Gavin Archer will pull out an invisible gun before shooting himself.

Mass Effect 1, Pinnacle Station DLC: At the terminal in your new home (after completing Ahern's mission), the Geth Pulse Rifle you can get is not the same Geth Pulse Rifle you can get in the main game. The model is the Avenger/Lancer model and you can equip mods on it. The real Geth Pulse Rifle you get in the main game has its own model (carried forward into ME2 and ME3) but does not allow you to equip mods on it.
Mass Effect 1: Enemies can sometimes get stuck in walls or on top of ceilings. This is typically seen on side missions and can be fixed by exiting and then re-entering, which will reset the enemies' positions.
Mass Effect 1: Conrad Verner assignment outcome is bugged for Charm option. Selecting the Charm option checks both the Charm and Intimidate flag in the save file, resulting in him thinking you pulled a gun on him when you meet him in ME2. Some dialogue in ME3 makes note of this and "fixes" it to a degree.
Mass Effect 1: Paragon-only sidequest (drugged-up hostage scientists on Chohe) is apparently bugged. Even if you complete it successfully (and Hackett mentions that you saved everyone), the game still sets a flag where it thinks you failed it when you import into ME2. See news broadcast where Sirta Foundation plans to shut down since it could not recover from an attack on one of its facilities. I believe this is "fixed" in ME3 with a codex entry if I recall.
Mass Effect 1: Shiala sidequest is apparently bugged where on import into ME3, Shiala (The Consort) has to leave the Citadel because of bad press. This is "fixed" in the ME3 Citadel DLC with some dialogue with Shiala during the Casino mission.

If it is true that Bioware no longer has the Licence to the Unreal engine, I don't think we'll get a remastered trilogy.
That being said:
ME1: Noveria/Peak 15
Textures aren't loaded/lit on certain ATI cards. The cause seems to be shader related. video
ME1: UNC: Listening Post Alpha // Planet Nepmos, Erebus system, Styx Theta cluster
Frame- rate drops to unplayable levels after leaving the rachni infested mines. This also affects the main menu.
Wiki claims this only occurs on consoles, but I have the same problem on my PC. <Fixed by a reload>
The wiki also states:
Approaching the rachni colonies for a second time while the corpses are still visible may cause a crash to desktop on the PC version. It appears that the trigger attempts to respawn the corpses.
(Hasn't occurred to me yet)
For some reason I only found the wiki's trivia section as reference to these.

Don't have a vid but when enemies climb on top of cover (not over it) they occasionally float. On the last part of the AA run where the banshee is there's table to the far right they climb on it and then float for a while. The same thing happens at the collector base for me, Garrus loyalty mission the backgrounds are all blacked out for the cinematic in the car. XBox360

ME1: In the fight with Benezia after Shepard gets back up from being knocked to the ground again Shepard may remain stuck and you'll have to reload the scene before the fight.

Man, that spot in the cockpit is so annoying. I always have to tread so carefully not to hit it.
From the top of my head, it's also really easy to get stuck in the area in front of the Purgatory club, if you listen to the 2 turian cops. There is a third turian, wandering around and if you stand right next the two stationary ones (the ones that talk) and the wandering guy walks into you, he gets stuck and so do you.
Also, Miranda's and Jack's slides do not trigger during the destroy epilogues unless you romanced them (here is a fix for it)
ME2: Really minor bug where TIM's face glitches through the level architecture (barely visible). Video.
ME2: N7 Mission in the corrupted Hane Kedar facility on Capek (where you have to destroy the factory). It's really easy to "run up the crates" and get stuck on an invisible plain one level above the ground. You cannot get back down and have to reload. This can also happen in the swamp mission on Tarith (although I haven't encountered it that often there).

ME1 Cerberus UNC mission: admiral Kahoku's body will dissapear if you enter the buildings in the wrong order or if you interact with a certain terminal. This makes the mission impossible to complete. Loading an earlier save is required.
ME1: The Mako will sometimes disappear if you wonder off to far from it and enter a combat situation.(can occur on Feros)
ME2: Jericho Station, Strabo System, Eagle Nebula. The lab room with the laser experiment - watching the laser hit the final point will crash the game. Need to look away from the beam to avoid this.
ME2: The camera will sometimes get stuck at a weird angle where Shepard will look one way and the aim will point at another way.
ME3: Ashley's marksman skill is still bugged.(she will stay stuck in cover activating it continuously)
ME3: Getting stuck on the ladder of doom. There is one on Eden Prime, one on Omega DLC and one in Leviathan DLC (the planet with lots of harvesters). Moving the camera at a certain angle(down then up) is the only way to pass but takes a few trials. Very annoying.

Are you sure about that Mako thing? I never have it happen to me. Added all the others with certain sources (wiki pages and a relevant thread). The last three were already on the list.

Have we had the shep / liara bug where their heads track glyph as he moves around the cabin?
There is one about weird neck twists. Is it what you refer to?
