First I would like to say good job on the game. I haven't bought anything remotely related to EA in over five years but I caved for Inquisition and I am not regretting it. Thankfully there is nothing in the foreseeable future coming that I want to purchase from EA. I consider it a momentary relapse.

With that said, I would appreciate it if some of the annoying bugs that should be easy to fix would be addressed. It's killing my OCD that some of these minor things just are not working right. I'm currently playing on the PS4 but wishing I went with the PC since others can fix your game for you:

1) Fix the last mosaic piece in the Hinterlands. Seriously. The eleventh looks bugged since it is up and behind the edge of the map near what looks like an incomplete level design, but at least it can be reached. Fix the coordinates of the last piece so it can be obtained. It's currently sitting in an inaccessible area and it appears it should be near the wolf cave.

2) Fix the missing piece of the mosaic for the Archdemon in Skyhold. It shares a plot ID with another piece.

Note: Both of these have already been fixed by the PC community (but I'm stuck on the PS4). I'm sure they can help you with fixing these issues:

3) Landmarks in the Approach has a bug. The last landmark I had was The Last Step, which appears in Echo Back Fort where you fight Servis at the top (little yellow pyramid shows up on the map here also). The fact that this was my last landmark may be important as I believe this specific one has an issue. I was able to claim it, and the landmarks quest is completed. Then the flag disappears if you walk away and coe back. If you look at your map, a new ? appears just north of the fort, and below the fade rift in the north. As I walk to it, it immediately changes to an X on my map indicating a landmark. There is no X where I actually claimed it in the fort, so I think some coordinates are off. This wouldn't be an issue, except for the fact that now the "Landmarks in the Approach" journal entry has now vanished. It doesn't show as an open quest, and it is no longer listed now underneath Collections > Landmarks in my journal. Reloaded from a save a few times just prior to claiming this last landmark trying to change things up, but it was the same resultevery time. Now I'm missing a quest and its driving me nuts. It almost made me quite playing all together because I was convinved my save has been permanently damaged.

4) Provide other ways to get decorative gems to fill Hinterlands requisition. I didn't get enough drops from enemies before clearing out the Hinterlands and now I can't get anymore. I'll never get to the other Hinterlands requisition now. Either leave some enemies roaming around, or have some merchants sell them.

I just reached the Exalted Plains, and thankfully the 20/21 region bug has been fied in the latest patch, otherwise I wouldn't be able to continue playing. So thanks for fixing that one at least so I can continue making my way through the game.

This is not related to that landmark because I have exactly the same problem and the last landmark that I claimed in WA was  "Ritual Tower" and at that moment the entry in my journal was stating 14/15.

 

Just to be sure I reloaded my previous save and set that collection as the active quest, therefore when I claimed Ritual Tower on the top right corner of my screen I could see clearly the message "Landmark in the Approach - Quest Complete". But then the entry disappeared from my journal.

 

Therefore I suppose that the bug is just the entry who is deleted from the journal, because the quest itself is considered completed.

 

I would also add that I didn't have this problem in my 1st playtrough, just in the current one.

Also fix the missing Amulets of Power that vanished in favor of "fixing" the loot exploit.
