Okay according to the latest patch notes, they fixed problems which caused Perseverance to be broken.
But...
Still broken for me, so can't get Cullen to re-appear (which means the Samson quest also broken).
I know the bug is related to Sera's pranks quest.
Has anyone found away around this? Pretty please - pretty going to have to start the game again.
Maybe the patch doesn't fix the bug but prevents it from happening on playthroughs where you still haven't done Sera's pranks.
If it can be of any help, to prevent it you should postpone pranks until after Here Lies the Abyss or Wicked Eyes, Wicked Hearts (credits to Darkamaranth for pointing that out when I asked some time ago).
And that can be a pain if you're romancing Sera.
