I keep posting but I'm not getting help, some (but not all) of my decisions are in the Keep but my 'Inquisitor' simply is not there no matter how many times I reload the saved game. What do I have to do, I can't bare the thought of starting again.
This is really upsetting me, please help me!

You can always rename a default.
Try going to a different area and swapping some inventory stuff around and then go back to skyhold.

I do that all the time but he won't appear there :-( . I don't know how to rename a default.
