Depends on the kit and map, plus how long since I last played.  One or two isn't unusual five plus is rare but not unknown.

 

Farming grab game 80ish been a while so I don't quite remember.

In Gold games with Geth Scanner....6/10 games....easily...I almost as soon as I see the chance to do it..I am already running for the grab...Silver..( Yes, I do play Silver)...I can get up to 10 grabs a game  because the AI isnt so aggressive.

I just got a gold grab medal there vs Cerb. JRandall witnessed it happen. So the answer to the OP is yes, sometimes at least.

I find it easy when using Infiltrators. Enemy covers and you go invis to make sure it wont go away. Also there are some maps with just the right spot to do that, those places where the only way for enemies to get in is by covering and jumping.

Probably not all that often, considering that I really enjoy doing them. 1-3 per game is more frequent for me.

If you're trying to complete the challenge for Combat Mastery, just grab-farm on Giant in the control room (do a Cerb bronze/silver solo with a Kroguard or AIU or something and just wait for them to come into the control room and take cover behind one of those counter thingies- can easily get 20-30 in the first 2 waves)

i Possibly when I was grinding the grabs challenge, but I haven't completed it since.  I've recently made a conscious decision to try and complete it a second time, but I never get more than 3 per game playing gold.  More often than not I'll run up to some cover and hit A, instead of B, vault cover, and get twatted.

Lately, in almost every game I play, since I play almost nothing but themed solos (and have to redo them a lot). Just got Over-Cover Grab 3 from Combat Mastery, can't remember the last time I got it without consciously investing in it (read: farming).

 

In team games, I rarely think to try it, and when I do, chances are someone will kill my mook before me, or I'll derp up the grab and end up killing them myself the conventional way. Methinks 250 grabs is a ridiculous requirement in normal circumstances; 25-50-100 would be about right, compared to other stuff in Combat Mastery and elsewhere.

Most of Combat Mastery is pretty ludicrous. You need 1,000,000 melee points, but only 300k with grenades. I tend to get Grenade Damage 3 the day after completing Combat Mastery, and then the rest takes weeks or months (because scrub reasons, in my case). You'd think that 1M melee points would balance with about 10M grenade points. Maybe.

I've completed combat mastery nearly 15 times (need 28 more grabs) without farming for any of it. Pyjaks.

The problem starts once you play with good people who kill enemies before you get a chance to grab them. Snap Freeze, Stasis and so on are also annoying.

When I farmed grab for combat mastery otherwise once in a while usually at least 1 or two grabs per game

I finished Combat Mastery about 2-3 weeks ago and ended up getting Over Cover Grab 1 about a day later. So I got a few 5-grabs medals that day.

 

 

Toned it down a bit and try to get at least one grab per match.
