There are several instances throughout the game where it tells you you have acquired an agent yet they never appear in the perks list and give no benefit? Will this ever be corrected?

There are several instances of this happening but the one that comes to mind is Duke Gaspard's Fereldan mercenary captain towards the end of Wicked Eyes and Wicked Hearts. You can use the Underworld Knowledge perk to recruit him and you get an Agent Acquired pop up and everything yet he does not show up in the Perks list nor does he affect War table mission times.

If you use the Underworld Knowledge perk to recruit Servis as a smuggler he also doesn't appear, afaik.

There are two dialogue options to recruit Servis, and the one that requires underworld knowledge does not work. You don't get him as a smuggler, and no war table operation is unlocked.
However, if you pick the other option, the one that supposedly recruits him as an informant, you get him under Leliana's tab plus a war table operation about *smuggling*.
