Enemy in front of my character, with my character turned toward enemy, camera behind me facing directly toward enemy, hitting immobiled target or target attacking me or otherwise stopped / busy in an animation. I do not attempt to attack moving targets with it. If I am using it to lunge across the screen, I follow up with a combat roll and attack cancel x2 before deciding to either move, CR, CB or MB next. Especially true if I am whiff-missing after the PS lunge. However, the point of the lunge wasn't necessarily to hit, but to glide across the screen immediately.
I don't have video of you missing, so I cannot tell what you're doing wrong, but I do know what to do to make it right, and I listed the qualities above previously.
On host rocks, off host sucks. Hard target via cycle target (default tab on keyboard) is ideal, though not always an option for time (helps a lot though for bigger / boss targets, can cycle on approach). Camera orientation always matters. Character orientation matters. Character positioning matters. Target positioning, vector movement and direction matter vs character vector alignment. Range matters (again with positioning).
PS + other Melee skills are awesome, but you do need to learn to use them. I don't know how you're using them and failing at them specifically, but those above are all things I've learned to take into account and adjust for. If you want to make a video of how you're failing at it, I can point out why you are, but it is probably best if you begin to learn the habits of manually orientating your camera toward your targets and using cycle target for a hard lock, while adjusting for spacing, movement and timing.

The thing is that I've never had to use the target locking before to make it work. It was quite a fool-proof way to deal with enemies before, even off host. The only thing I changed is switching to autolook the mouse. I'll try without and see if it works differently, then.

OK, I just tried it out and it looks like toggling on mouselook screws up the tracking of Pommel Strike.
Playing the old way RMB spam: Working as intended
Using mouslook: Not hitting stuff right in front of me.
I also had the Katari end up in a Jesus-like pose trying this out. It could be cancelled by a combat roll. Looks like a bug - at least on PC.
tl;dr: Everybody having trouble using it - try to toggle mouselook off and see if it helps.

I wouldn't be surprised if it broke tracking for other skills as well. Good job sorting it out.

I also noticed that with the autolook toggled on, the Katari wouldn't do the lunge towards the target. So I guess tracking is completely disabled for PS.

A little off topic, but I've been getting the "Jesus" pose a lot on my lego. Sometimes in the middle of a skill rotation he just goes statue. Jumping or using another skill cancels it.
On topic: always takes me 2-3 swings to connect with PS unless situated almost perfectly.

I saw it the first time yesterday - it looked like a revive im MEMP on really laggy hosts. But I was soloing. On host lag confirmed?

Pommel Strike worked finew when I played the Katari yesterday. Its very odd sometimes it works as intended and other times you have to use Pommel strike 6-7 times in order to get it to work
I'm pretty sure my issue is a PC thing with the autolook. When it didn't work for you, did the character do a lunging attack?

Haha, that's me on any kit! I think I've discovered a new class: Fadewalker. My abilities are seeing enemy HP, resistances and status effects.
