Try to switch to another character (companion). They shouldn't be in combat.

It's an old bug.

Ya it's an old bug, especially when dealing with dragons or reloading an old save while in combat.  At least thats the only two times I've came across it (on ps3)

That's weird because it rarely happened this often. When I switch it still doesn't work.

 

Edit: Reloading seems to help.
