1) How do I load a keep without DAI choices? It seems that it is always there. Do i just choose nothing?
2) there are some 'extra' DAI heroes that I have already deleted but still appear in the Heroes in DAI. how do i delete them? understand that I can delete those heroes from DAO and DA2 but not DAI. is that the case?
3) and why did it says i have 4 playthroughs for DAI when i only have 1?
1) You DAI choices doesn't matter if you want to export a new world state to the game. If that's what you meant ?
2) Yes. You can delete heroes from DAO and DA2 but not yet DAI. So any character created ingame (even deleted) will appear in the Keep. The option to delete them should be added in the foreseeable future according to the Keep devs.
3) Any new character correspond to a playthrough. So if you created several characters and then deleted them before your actual playtrough, each one will appear as a particular playthrough.
