Team Composition Help (Noob)

I have just started with RPG games genre, DAI is my first rpg title.
I have a bit of trouble in deciding my team composition.
I am playing on normal difficulty and plan to play on hard on my second play through.
Being new to DA in general, I have been reading/watching all sort of guides, tips and tricks, YouTube Videos and all,but unable to find to find a good composition for team
I am KE Mage (main character) and don't like to switch to other character (I let the AI do its job).
So tell me the perfect composition (keeping in mind that I will/may like to use the same composition on hard too)
I am bit inclined to use following characters:
-Blackwall as Warrior Sheild and sword track
-Cassandra/Sera (any one of these 2 shld be part of the team)
If any of them hampers the overall balance of the team then I am ready to leave them (although a bit reluctantly) .
I am level 12 midway into the game and didn't have much of a hard time till now (although I do died sometimes but overall it has been fun)

As a KE you will be fine with most anything you try until the DLC.
If you like Blackwall then keep him. He is quite powerful and I like him too. I don't normally run more than one warrior and my most recent playthrough is 3 mages and a rogue. As a KE you are basically a tank, so keep Sera, especially since Sera inspires certain scenes in quests that are otherwise not possible. The other slot can be any of Varric, Cole, Solas, Vivienne or Dorian. Any of them work but I do recommend Solas on a first play-through since he has some interesting insights into quest objectives that you will want to see.
I hope this helps. I am trying to be vague enough not to spoil anything while showing reasons for my recommendations.

I'm playing KE mage on my hard playthough and I have Blackwall as my tank, Sera as an archer and Solas as rift mage with fire skills. But I like to sometimes manages my team and I usually play in tactical view. Anyway if you read guides you probably know already about prioritizing companions' skills (for example I have all tank taunts skills on preffered), thats useful. And sending tank first in a fight so he grab all aggro. I also order Solas and/or Sera to take care of ranged mobs.
But I think there is no such a thing as "perfect composition".

The composition you have recommended will it work on hard difficulty too?

You will be able to that team to Nightmare in the DLC. Just spec Solas as support and make sure he get Dispel before the DLC. Dispel eliminates poison, barriers and the glyph mines that enemy mages put down.

Didn't knew about prioritising the skill (thanks for the tip) (I Just Prioritised War Cry on Blackwall)
Archers are real PITA
I am having bit of trouble in dealing them. How do you assign your companions to prioritize Archers or any particular enemy
So far I didn't reached the Dragon part, they look scary to slay from the outlook.
Instead of solas I am bit more inclined to get in casandara, will it work too?

I also have Barrier preffered on Solas (or Dorian, I sometimes switch them) but AI does medicore work on that imo, but if you don't want to managing that it's rather useful.
I haven't play with Cass instead of Solas/Dorian but I think if you spec her to dps (two tanks are boring, too long fights imo) she sould be fine. I'm planning a run as a tank with Vivienne, Cass and Cole and I hope that will be viable. I'll just respec Cass for 2 handed.

If you want Cass instead of Solas that is fine. Just make sure she has Purge and spec her into Wrath of Heaven. Wrath of Heaven is an AOE stun and if she then purges on top of the stun then the Nightmare combo will fire for quite a bit of damage. Give her a two hander to make it even sillier. This will require a respec to be optimal. Cassandra can be a very good support tank.

Why some of my character are level 11 and rest are 12?
I've wondered that myself, no idea why.
What's your playstyle? Team composition for a first playthrough (PT) for me is all about the story. I lucked out and randomly picked Cass, Sera and Iron Bull and those three have among the best backstories and best banter, and by "best" I mean closest match to my own tastes. YMMV.
For sheer DPS might mid to late game, KE/Sera/Cole/Solas, is hard to beat. Seriously, you could solo with KE on Nightmare, though duo with either Sera or Cole is probably more fun.
I'm not sure why you'd want to play the same team on a second PT. Is it because of the practice value, use the first PT to get a feel for how to play the team, and the second for optimal combat? But you said you never switch characters for combat, so I'm puzzled. I'm not saying it's a bad idea, I just don't understand why, and thus, can't really offer useful suggestions.
You also don't have to stick with the same team for the whole PT. I'd swap at least one character out for each major quest. Sometimes that's a mistake story or roleplaying (RP)-wise, but such mistakes can be fixed in subsequent PTs.

Well even I am discovring more about my taste (So far I have no fixed, as such, playing way).
I inititall played as KE doing barriers and other spells from far away on my DW Rouge and warriors (2 of them), but after unlocking KE I am very close to enemies & slashing my way (I feel like a tank now, thanks KE)

I said I dont want to switch team beacause I wanted to get a settled team which can stand against any enemy, although I have now realised that one might have to change players as per the each battle demands
. Apart from that no other reason for not changing the team.
Furthermore I dont use tactical camera because I have little to no idea about the other characters powers and have little knowledge on how to instruct other part memebrs on where they should stand or where not.
Uusally what I am doing now is sending my tank to go aggro and me (as KE) fighting enemy from close range and sera (Archer) and other character (randomly picked) to stand at far do their own thing
One more thing I want to ask, I have one level 3 schematic for weapon (which I belive is the highest tier), are the weapon stats influenced by our level for instance I am level 12 now, if I craft a weapon on my current level will it get obsoloute by the time i reach a higher level (say 16)?

Yes, it will go obsolete, but not for the reason you are thinking. For a given schematic, the quality of gear is influenced only by the materials you use to craft it. There is only a loose association to level, in that some of the best schematics and/or material are supposed to only be available to high level characters -- but there are ways around that. For example, there are some Perks that give you higher level schematics and materials earlier than you would normally find/earn them.
Rune materials have the closest association to level, since the only way to get the materials is to kill high level enemies. Pure essences from high level rifts, for example.
If you have a T3 schematic now, but only enough T1 materials to craft the gear, it will be obsolete sooner.
That doesn't mean you shouldn't use the T3 schematic! Go ahead and fill it with the best materials you've got, even if they are mostly T1. A schematic is reusable and you can craft new gear when you find better materials. Unless you hate crafting, you can replace your gear once or even twice per experience level, depending on how much time you spend farming materials.
