Not sure how, kind of a big deal if you ask me.
Apparently I'm earning renegade points which aren't being acknowledged via those annoying pop-ups just for doing nothing.
First encountered- 0 Renegade points after Saving Mordin's assistant. Not 1. Next rooms: clear out Vorcha, cure the plague. +9 renegade points, +4 Paragon.
I've replayed the encounter 6 times now saving as near as possible to the last save point.
Unless killing Vorcha is a Renegade action, or if curing the plague is a renegade action, then I'm not sure what it is.
This has to be a bug, or a really bad plot device. Given the still unfixed bugs in Mass Effect I have to presume it's a bug.

I ran into the same problem. I think you need to leave after the mission is over instead of staying in the clinic. I did that and it seemed to work. Hope this helps you out.

I'm just guessing based on my own playing, but I think the way you earn Paragon/Renegade points has changed slightly. I've been getting a mix of both points after a single conversation, so I think they're based on your individual responses, not just the ones with red and blue letters. That's just a guess, though.
