2) Shepard did not take the renegade interrupt to shoot the model of the Citadel. So who shot it? I didn't see anyone shooting it. (I had Ashley and Garrus on the squad.)
No. Clone Shepard doesn't have the baggage.

When I had Wrex in my squad, Garrus shot it, when I had Garrus - Wrex. I had Tali with both of them. Maybe Ash complicates things?
CloneShep is just that: a clone, so they don't have any of the RealShep's cybernetic implants, thus no possibility for the scars.
I always thought that It would have been a nice touch if CloneShep would have had the face the player used in ME1 (minus the scars of course) regardless of any changes the player may have made at the start of ME2 or ME3, but I'm guessing there wasn't a separate tag for that along the way.
Another question:
I take you still get "Team Mako" and "Team Hammerhead" even if you have the minimum of 3 squadmates (Vega, Liara, and EDI)?
