I crafted the brutal pommel and now its in the upgrades tab of my inventory, how to i use it on my sword?

Visit a weapon upgrade station in the smithy of Haven/Skyhold.

Find the Modify Weapons station, select the greatsword, select the greatsword's pommel slot, select the new pommel to add.

It says that i cant upgrade it. Its a great sword, does that matter?

Some items just can't be upgraded. Try a different greatsword.

Yes, not all weapons are upgradeable. Many rare and unique weapons (blue & purple) can't be upgraded.
Crafted weapons always can, though.
