Doing my second playthrough, on ME 3 now. I think the Squall Theory explains everything nicely.

 

In the opening scene where Shepard is talking with the committee, she dies during the Reaper's initial attack on Earth. Think about it: no way is old man Anderson going to be running and jumping like Captain America going from building to building. She fantasizes about killing Udina, watches the Asari ******'s planet burn, shoots Ashley, and even sees Mordin flip on the Genophage along with whatever love interests are renewed, rekindled, or started. Pure and total wishful fantasy.

 

The rest of Mass Effect 3 is merely the final moment's of Shepard's life, before she finds comfort in the Embrace of the Goddess.
