After a long exposure of the red lyrium that was infused in her sword back in DA2 wouldn't it had made sense for her to turn into a Behemoth instead of a giant size red lyrium stone? It would of made perfect sense for her to return to Inquisition as one of the main antagonists that is working with Corypheus.

Weren't Templars that were turned into killer gems force-fed red lyrium?

this would imply

A. BioWare plans ahead for stories and characters

B. They aren't making the lore up as they go

It might be because of how Meredith suffered exposure: She was not ingesting red lyrium nor did it seem like she was having bits of it jabbed into her like the Red Templars were and had.

Meredith's exposure came through non-skin contact and likely also from channeling Templar abilities while wielding the sword which may have had a strange effect when combined with the red lyrium in the sword. It is possibly she may have touched it without gauntlets or something but I think contact of that sort was minimal. It was deterioating her mental health primarily because of her proximity to it. Her turning into a statue possibly came from using her powers and combining them with the sword which may already have been destabilised hence why it shattered.

The lyrium idol may also have been different from regular red lyrium.

It's mentioned in the Emprise that having the change take place too quickly just turns you to solid red lyrium. Clearly, that was what happened with Meredith.

I would disagree on several points, but think of it as using the energy too fast - she burned up!

It may be that the lyrium idol was designed to basically just quickly transform people into red lyrium.

Because the devs don't pay attention to the details in the lore they themselves have written.

I think it's like b10d1v stated
Like how a nuke explosion can turn the ground into glass(I forget what it's called)
Maybe she accelerated the process by exerting so much power/energy?

She had the idol for years, though. So it wasn't a quick process.

She didn't use it, though, and she was experienced with dealing with lyrium. It may be that the process only started once she converted it into a sword.

More likely, Bioware just didn't want to get accused of recycling the ancient rock wraith as a final boss.

I dunno. It didn't seem like Bioware were worried about recycling-accusations at all in DA2.  If the cave and the building interiors in the game is anything to go by.

Every Red Templar, even in Inquisition, doesn't turn into a behemoth, but Meredith should?

Behemoth didn´t exist before. DAI needed some cool gigantic enemies, coz reasons. Exhibit A: Giant dwarfs.

They recycled environments sure but I don't think they wanted to recycle their own bosses.

Or the idol was designed to be able to draw off it's power without suffering any severe physical effects. Barring the final transformation of course but that might be inevitable.

I remember a character in the game mentioning something about Samson's armor being the key to his power and exposure to it made him more and more corrupt. I imagine Meredith ran into something similar. The only difference is that Samson was ingesting red lyrium at the same time—Meredith was not.

I am not sure that at that point "Bhemeoth" was even written.

Most if not all red templars were ingesting the red lyrium. Meredith was just close to it. Hell Bartrand had a chunk of it for years but all that happened to him was going insane.

It could be fun to have Meredith as a talking statue like the one in DAO, but batshit crazy. Blabbering her rage.

To become a Red Templar in a short time you need to ingest Red Lyrium. Meredith didn't do that, that's why it took years for her to become mad
