Just finished a 205 hr play through on XB1 - no meaningful bugs since patch 2 - I found Jaws of Hakkon dlc very enjoyable, extra tough region.
Though your mileage may vary.

Not all have been fixed yet. A few examples:
Stonefist: detonates combos but does no extra damage. So if you SHATTER a frozen enemy with stonefist you will not get extra cold damage from the detonation AND the enemy will no longer be frozen, although you still get the spirit damage from stonefist.
Dispel upgrade (transmute magic): partially bugged because bonus damage and bonus barrier only triggers when dispelling enemy barriers, even though the game suggests it also triggers when dispelling status effects.
Rejuvenating barrier: partially bugged because it only works on yourself. (no stamina/mana regeneration for your party members as suggested in game)
Hidden blades: acts as a PRECISION detonator, even though the game doesn't say so. This actually makes hidden blades more useful.

The broken perks are still broken. Evidently, their attention in the single player campaign is limited to eliminating exploits.

I don't know if the OP is for real, but why the unhelpful sarcasm?
You know the seven patches are not limited to just closing unintended loopholes
.
Has to be said, as I've become accustomed to playing the game as it is, i've not noticed any perks or skills not working because I would not notice the absence of something not doing something (if you see what I mean).
I have avoided a small number of skills in multiplayer as I had understood they were not as effective as intended.

Deathblow is broken too, it consumes 50 extra stamina in many cases.

And yet some bugged perks were fixed.

Also, they have broken the game in new ways. For example, about half of the available amulets of power in the game were removed. Bioware won't fess up if this was intentional or not.
The game is still playable. It is supposedly more stable now. And there is the option to store items and tint your armor which is nice.
If I can make a recommendation. Hold off. Don't return yet. Wait until the Black Emporium is available. It'll save you countless hours and many boring Hinterland chores.

Decided to replay using tempest (only my 2nd playthrough, cus as insane as it sounds when I say it out loud, had to leave DAI for some other things & have been seriously out of the loop), and noticed that flask of fire doesn't work on focus like it did on my 1st playthrough for Sera... That a recent fix (cus I kinda liked that particular loop hole)???

This exploit was fixed in one of the recent patches, yes.
