I recently completed DA2 and about 4 days ago all my custom heros were available on the Dragon Age tapestry.  Today all my custom heros have disappeared from both Dragon Age Origins and DA 2.

All fixed, some odd reason my Origin account used an old account to sync my games includding DA Inquistion, everything back to normal.
