Seriously, though, modeled after Europe or not, Europe didn't have magic. I betcha you can buy a potion at Wonders of Thedas that makes it so your **** don't stink. Oh, and it probably looks like a rainbow, too. And it can used as nug food! Ha, suck it Dalish, missing out on tranquil potions for pretty poo cuz you wanna live in the woods and be all kumbaya hippie happy!

Just because it is visually similar to our past, doesn't mean everything about our universe has to parallel ours. Their technology and infrastructure doesn't have to match that of what Europe had in the 12th century. We don't know how we deal with their waste.

I was always disappointed I never got that bath I asked Josephine for when we got to Skyhold but the fact I could ask for one seemed to point to the fact that they were there somewhere in the Castle. Ancient Rome had very sophisticated plumbing but somehow the technology got lost after the empire receded. Tevinter is based on Rome but in their case may be the barbarians kept the buildings and the plumbing after they took over.
I'm pretty sure Dorian mentions something about Blackwall needing to get some soap. We had the same comment from Wynne about Alistair and putting him with dog so the smell could be all in one place but she did want to bathe dog, so I think personal hygiene is an issue more than public sanitation. Orlesians complain that Ferelden smells of wet dog but I think much of the time they are exaggerating in order to belittle their neighbours so I think a lot of Sera's comment could be seen in the same light. Plus if you are not used to a certain smell it does seem very pungent, whilst someone living with it every day barely notices it.

Ferelden may stink of wet dog but a campsite after Oghren had way too much to drink and eat is much, much worse.

The wiki suggests that Denerim's Poor Quarter lacks a proper sewage system (the reference is the Dragon Age table top rpg) which Sera likely didn't live in very long (due to her adoption by Lady Emmald) and I imagine that this is true in most major cities in Thedas perhaps with the exception of some of the Tevinter cities where they are more magically and technologically capable. What we saw of Val Royeaux was glitzy and sparkling, but according to the lore there are crowded slums there and I can't imagine that the authorities would bother maintaining a working sewage system in these areas.

Heh. In some medieval cities the poor would actually live in the sewers. They would be wading knee deep in human waste - in their own homes.
That said, is the objection of OP accurate? Pretty much. Do I want to see this in a fantasy game? Please God no. I like gritty realism, but there are limits. Sera, making her introduction while being entirely covered in crap, would make her presence even less enjoyable.
