As  regards bathing, if you are in Vivi's bad list and ask her if you and her could maybe someday (implying getting it on, yo) she'll reply with something along the lines of maybe someday if you bathe more often.  So clearly regular bathing is a thing if not doing so can be seen as an insult.  And one would think regular bathing would go along with a decent plumbing/sewage system (assumption only, don't bite my head off) that could deal with da poo.

 

I still like the idea of magical poo pills, though.

Denerim has a public well. If you're drinking groundwater, your sewage had better be routed through separate sewers or cholera would be a real problem. But then, Gamlen says that he nursed his parents through two bouts of cholera, so I suppose that Kirkwall wasn't big on sanitation.

 

That said, Paris's sewers date to the late 14th century, so it's possible that *somebody* might have boarded the clue bus.

 

(Sighs and reminisces about crawling through the sewers in Baldur's Gate...)

 

Maybe they have sewers in Val Royeaux?

Anyway, I doubt anyone wants to 'experience' or 'feel immersed' in the stench of Medieval cities. Next up every enemy you kill will loose their bowels and your PC will die at the age of 23 from dysentery or bubonic plague.
