The Virmire decision was contrived so that you had to choose when realistically you could have saved both.
I have to disagree with the bolded. I think of all the missions in the Mass Effect series Virmire was perhaps the most realistic in that Shepard couldn't save everyone. That is more often than not a sad reality of combat in the real world.
Had I lived during the American Civil War I would have been a Union man, both by place of birth and ideology (The ideology of the CSA was as abhorrent as Nazi Germany), but I think Robert E. Lee has a great quote that is relevant.

Yeah, but the way it is presented, you should be able to save both. Example, you go back to the bomb, save VS and fly away only to discover that Normandy has also evacuated Salarians from the planet. If you have time to save them, why not Ash/Kaidan?

One possible reason is the bomb didn't kill them, but they were overrun by Krogans and died that way. So they other groups were able to hold off the attackers or didn't have as many and were able to survive until Shepard picked them up.

Normandy picks up Kirrahe and he is with Ash/Kaidan if you go to the tower. But then again, we start delving into headcanon territory here. All I'm saying is that it does feel that Shepard could've saved both, especially if he went for the bomb.
I think devs intended for Shepard to go to the tower. Saren's grab has more impact there, VS gets a unique cutscene when Normandy flies away, Salarians are saved anyway.

The Salarians should have died if Shepard went to the bomb site instead, regardless of earlier choices in the mission. The setup could probably have also had some minor tweaks to make it clearer that whoever you didn't go to reinforce initially was going to be overrun, like a line of dialogue saying they had hundreds of Geth swarming their position when you went the other route.
Minor flaws aside I thought the outcome of Virmire was much better than the Suicide Mission, where assuming the player didn't skip content Shepard's team only suffers casualties if he/she makes tactical errors. I did like the mission structure of the Suicide Mission better however, since it involves the entire team. ME: Next needs some of that, if not the same flawless victory outcome.

Mistake or not, its the way it is. I too think that the scenario could've used a few tweaks to make the situation more clear (and making sense) but the way it is presented now Normandy saves Shepard, VS from the bomb site and the salarians from the tower while leaving Ash/Kaidan on Virmire.
I'd say the most realistic choice scenario was with those nukes in ME2 when you had to save either the colony or the spaceport.

If Mass Effect ends up having a decent showing this year, combine that with the Fallout 4 gameplay demo we'll likely get, I'd say this will be one of the best E3's ever!
Even if we only get a trailer and about ten minutes of info, it'll still be more than what we got in the last year or two. Aside from N7 Day and San Diego Comic Con, there hasn't been too much info given out yet.

Actually this is a rather rich year for gaming, announcements of many cool games.
Fallout 4 already makes me happy, still ME:Next is the top expectation for me.
I think those tiny sneak peaks we were having for BioWare and their staff on Twitter indicate we will get something at E3 pertaining Mass Effect for sure, we just have to wait to learn how much they have decided to show us.
Since there is nothing in the way, even DA:I - BioWare might get bold and give us what we ask for at E3 - both trailer and few minutes of in-game footage.
This is just my hunch, but I'm totally convinced it's a safe bet.

I'm actually more excited to see what they do with Rise Of The Tomb Raider. Now that Laura Croft has gone from lolteensexidol to an actual woman... I can't wait for it.
I loved their last game. Just wished that I'd have had more time to play it.
