I've exhausted all dialogue options and I've gotten far enough in the plot. I don't know what's wrong, and I really want to get this quest done. Is there anything I might have missed or is this just a bug? I've got a 360.

Try leaving Skyhold, doing a few minor quests, then come back to Skyhold and try again. That is when it triggers for me.

Well I have two saves. Before and after the Temple of Mythal. Neither are working.

That's weird. By then it should have triggered if you talk to her every time you return to Skyhold.

It's a tracking flaw in the character controller and the result is a conflict on the character position -game believes she is in two places. You must clear the condition manually by going to a save before the flaw or find her and try and clear it w/o markers.
Unfortunately, any NPC can see this error when improper sequences are triggered. It's not your fault, but that of core game design.
