So I cannot seem to find a definitive answer anywhere, but as far as lore goes(as oppose to what is happening on your screen) do all the squad mates take place in the missions? E.g. If I bring Kaidan and say James on every mission in Mass Effect 3 or Jack and Kasumi in every mission in Mass Effect 2, and so forth for the finite number of combinations in 1,2 and 3, is my story tailor crafted to fit only those two at my side, or from the outside looking in was my entire squad present and they just don't acknowledge it. Obviously for the suicide mission and I'll even say Priority Earth(since everyone is in the vicinity to talk to at some point) they canonized it as everyone took place in the finally. But Mass Effect 1 and missions leading up to the finales in the games its never really discussed this aside from the occasional "We sure kicked Saren's ass." Please do not count missions where squad mates can not be present as they have not been made available to Shepard yet.

There are no huge consequences to this but in ME3, if you talk to squad mates after the mission, they will have different responses, depending on whether they were physically present or just heard about events from the others.
Also, there are a few instances in the game where the actors in certain scenes depend on who you took on missions most often on average. One prominent example of this is during the ME3 ending, after the Normandy crash. Which people you will see stepping out of the ship depends on this (among other factors).

In ME3 Tali and Garrus discuss Horizon and the dead Reaper. Also the Rachni mission on Noveria in ME1.
Garrus says "just like old times" in ME2 even if he wasn't recruited in ME1.
There are quite a few of such instances, check the Little Things You Just Discovered thread for more of those. I don't remember them all, sorry.

I think from a lore perspective the entire squad is supposed to be present, but it isn't reflected in gameplay. In ME3 returning characters for example can reference missions in previous games that you might not have selected them for.
