This may not be the right place for this, but has anyone else recently had their nightmare difficulty glitch back to normal without input?  I just tried my platinum trophy run with only the Nightmare trophy remaining, finished the game at 78 hrs, and no trophy.  When I checked the difficulty afterwards it was set to Normal.  I didn't change it.  Has anyone else seen this?
