Has anyone else had a problem where the difficulty has auto reset from Nightmare to a lower difficulty on DAI? I just recently finished what I thought was my nightmare playthrough, or at least that's what I set it to. At some point in my 76 hours, the difficulty reset to Normal. I didn't find this out until I finished the game and the Nightmare Trophy and subsequent Platinum did no pop. Lo and behold upon checking the options, it was set to Normal. Totally disappointing.

I have heard of it before as a random error early in the game, but it is quite rare -lucky you!

I got it, sort of. I had changed my mouse settings. I didn't like the changes, so I clicked the reset button in the mouse settings menu (Controls, I think). Unbeknownst to me, this reset the difficulty in a totally separate menu as well.
I had slayed the Ferelden Frostback on what I thought was Nightmare, but I felt it was too easy. So I checked right after I killed it and saw it was on Normal. I was angry, but I only had to set it back to Nightmare and then reload an earlier save. I had to do the fight all over again, and it was definately more difficult. Since that time I had kept checking to make sure it was still on Nightmare, and it was.
So it only dropped down to Normal when I hit the reset button in the Controls menu. It never did it on its own.
Also, when I connected to Origin to get my achievements, everything showed up properly. So I was fortunate. Sorry to read about what happened to you.

Thanks for the support guys, I guess it means I just have to play though it again. Darn! I think it happened when I transitioned from clearing Emerald Graves to Western Approach, I thought the difficulty change was just because of the level cap thing. Oh well, I guess I'll roll with a Qunari Warrior this time, make The Iron Bull jealous.
